The report highlights that staff at the Liverpool service were over stretched, leadership was inadequate and they failed to recognise that the cost reductions sought carried substantial risk.
The report found that Liverpool Community Health NHS Trust not only failed in its duty to provide safe and effective services but it concealed this from external bodies.
The report concluded that significant and unnecessary harm occurred to patients.
